linux pmda: Add per-node CPU metrics
This is just a 'smart' derived metric from the per-cpu metrics that already exist. Also export a hinv.map metric that shows the cpu->node mapping.
The Node indom initialization had to be moved to the proc_cpuinfo file because it is initialized earlier than the numa meminfo metrics.
