pmdaCache Fix corner case in LOAD logic
Problem identified by Max Matveev.
When the cache is already populated, the LOAD operation needs to honour existing cache state and report any inconsistencies (and not load the offending entries).
Also added PMDA_CACHE_DUMP to pmdaCacheOp() to aid debugging.
