On-disk version change for MMV format adding semantics support.
This change adds the requirement that metrics must be designated as
counter, instantaneous or discrete when initially defined, and that
designation is then reflected in the MMV PMDA.
Since format was changing, and this was trivial, also updated the
size of the metric/instance offset field in MMV file so that it is
(consistently) a 64 bit file offset (same as the TOC offset).
